"This book stems from the lunchtime clinical and academic multidisciplinary
meetings at Barts and The London. Hence it is a collection of evidence
based articles on different aspects of management of IBD. The chapters
are short, succinct and simply laid out allowing for a quick reference.
Helpfully there are summarised learning points at the beginning of each
chapter with a manageable number of references at the end.
"The book is divided into different parts focusing on investigation,
current and future medical management, common clinical challenges, management
of IBD outside the gut and service provision. Of interest in particular
are the chapters that focus on current and emerging management of IBD
from the investigations (for example the role of capsule endoscopy)
to management with immunomodulators.
"There are 5 short chapters that deal specifically with the use
of infliximab and these are of great practical use (there are three
pages dealing with infliximab infusion reactions alone). Furthermore
the evidence behind alternative and complementary therapies, probiotics
and nicotine to name a few topics is succinctly presented and the role
of leukocytapharesis and genetics postulated. I suspect of greater interest
to the multidisciplinary team are the last few chapters that deal with
the provision of IBD service in the context of current state of the
NHS.
"The mix of contributors to this book is vast (from international
professors to the UK trainees in gastroenterology) ensuring that the
material is clear and readable internationally. It also helps to achieve
the Editors aim of the book at a range of medical professionals
from established and trainee gastroenterologists to specialist nurses,
pharmacists and dieticians.
"The downside of a compilation such as this based on the current
evidence is, if any, as the editors themselves freely acknowledge, it
will soon become out of date and if to succeed will need frequent updating.
That said it is a fantastic book, for a trainee at least, to have in
a busy clinic or on the ward which will also provide references for
further reading around the subject at a more leisurely pace away from
the busy workplace."

The pocket consultant is aimed primarily at those embarking on
a career in gastroenterology.
The style is concise; tables and bullet points abound. This makes for
an ideal quick reference point in clinic or on the ward. Lists of differential
diagnoses, tables of scoring systems, and investigation algorithms are
all displayed clearly. Answers to most commonly asked questions can
be found easily.
'Each chapter outlines the basics of a topic in the sort of detail necessary
for MRCP. Common conditions are then expanded on in sufficient detail
to appeal to junior registrars and above. Absorbing the relevant chapter
prior to a specialist clinic is likely to see you safely through most
of what comes through the door. What it wont help with is the more complicated
cases. More experienced clinicians may find it lacking the subtleties
that they would need to direct the investigation or management of the
more complex clinical conundrums.
'Having said that this book is for junior registrars, one senior consultant
keeps a copy to hand for the useful addresses section so there really
is something for everyone.
'Overall I would strongly recommend this book as a starter text for
senior SHOs and junior registrars. Those more experienced will also
find it a useful revision tool. My only quibble- it doesn't fit in a
white coat pocket. I suggest it lives on the office shelf.
